Summary
Description
The rate of delayed transfers of care from all NHS hospitals per 100,000 population aged 18+. A delayed transfer of care occurs when a patient is ready for transfer from a hospital bed, but is still occupying such a bed. A patient is ready for transfer when: [a] a clinical decision has been made that the patient is ready for transfer AND; [b] a multi-disciplinary team decision has been made that the patient is ready for transfer AND; [c] the patient is safe to discharge/transfer. This therefore measures the ability of the whole system to ensure appropriate discharge for the whole population passing through hospital and is an indicator of the effectiveness of the interface between health and social care services. Description
This indicator measures the number of adults aged 18-64/65+ per 1,000 population that are assisted directly through social services assessed/care planned, funded support to live independently, plus those supported through organisations that receive social services grant funded services. The indicator will be age standardised and, if possible, adjusted for likely needs for social care services using needs-weighted population data produced from Relative Needs Formula (RNF) allocation calculations. Issues remain on potential double counting between assessed services and grant funded services and within grant funded services that need to be resolved. Number of net increases in dwelling stock over one year. The net increase in dwelling stock over one year is calculated as the sum of new build completions, minus demolitions, plus any gains or losses at the same address. Source: Communities and Local Government (CLG) Publisher: DCLG Floor Targets Interactive Geographies: Local Authority District (LAD), County/Unitary Authority, Government Office Region (GOR), National Geographic coverage: England Time coverage: 2006/07 to 2007/08 Notes: The definition of dwelling (in line with the 2001 Census) is a self-contained unit of accommodation. Self-containment is where all the rooms in a household are behind a door, which only that household can use. Non-self contained household spaces at the same address should be counted together as a single dwelling. Therefore, a dwelling can consist of one self-contained household space or two or more non-self-contained spaces at the same address. Guidelines: Good performance is typified by an increase in numbers of net additional homes.
